I believe Jordyn is getting her tonsils out. My guess is that the "secret" is that Josh and Anna are expecting another boy.
TLC synopis update:
Jim Bob and Josh are hurting as trainer Steve puts them through their toughest workout yet. Meanwhile, Jordyn gets her tonsils removed, and Josh and Anna go in for an ultrasound that will reveal the baby gender, but only one person will know the results.

Yes. I am sure your correct. The preview said about the tonsils and they were going to find out the gender.

Let's see...the Cake Boss knew the gender of the first child, and the sky writer knew the gender of the second...maybe the doctor will tell the parents this time?

anyone else notice - when Jim Bob and Michelle were explaining the tonsillectomy to Jordyn in their room, it looked like she was wearing pants? Or am I hallucinating?
I switched channels for a while when the little girl went in for surgery. An old episode with some of the older girls having dental surgery turned my stomach.
Is there something wrong with the hair on Josie? The top part at the front doesn't seem to be growing in - or maybe it's the way the hair is pulled backed with a clip. It makes that area look almost bald.

They pulled 1.1 M viewers and a .4 rating for this one. Must have pulled some viewers in to see the non-secret secret.
One thing that really bothers me about the Duggars is that they don't have any continuity of care with doctors... Maybe it's just because I work in the medical field but every time they go to the doctor, dentist, etc it is a completely different doctor and not even from the same medical group...
I was surprised that Anna is using a midwife from Ft Smith. I would've thought she would have used the one that Jana and Jill "work" for.
I thouhgt Josh and Anna were using the midwife in Ft Smith so that Jana and Jill could be on the birth team.
My guess is that is the one that Jill is apprenticing in/with.
Public record in Arkansas shows Jill apprenticing with someone else. I believe her name is Venessa Giron.
Sharla is correct. I believe Ms. Giron is based out of Fayetteville, so she's essentially local for Jana/Jill.
Fort Smith is an hour plus some away from Fayetteville. Where they live generally draws better doctors and health care. Fort Smith has no birthing centers. They would have more choices in NWA. It doesn't make sense for them to come to Fort Smith. (I live 45 minutes from Fort Smith)
Maybe they didn't pay for Mack or Mike and Jill's midwife won't see them. Josh would be like - "You got to deliver MY baby....on television. That is payment enough"
I'm sure it's rude to pick apart how people talk, but why does Michelle have to talk about Jordyn being "fearful." I can't imagine describing my toddler/preschooler that way, I'd just say scared. or worried.
